This webpage includes two forms:
- Census: Rates are based on the age and gender of your employees.
- Physician Survey: We suggest that you and your (key) employees indicate their important physicians. We can check the plans that they are affiliated with. This allows us to check whether changing companies will result in any disruption.
We need your feedback to design the best plan for your needs. You can get high quality plans with low office visit co-pays and low deductibles, though these are sometimes costly. Alternatively plan with higher co-pays or without prescription benefits costs much less. HSA and other high deductible plans cost less by allowing you to partially self-insure for large claims. In order to run quotes for you we require the following information: Health Insurance:
- Census: Will all your employees enroll? Do you know whether any will cover dependents (spouse, children, and full family)?
- Objective: The least expensive plan? A plan to cover specific doctors? A different plan for employees? Your feedback will clarify these questions.
- Are there any specific doctors you want in a plan? A survey form is attached.
- Is out-of-network coverage important?
- Is your priority plan cost, cost to use (doctors office and hospital), or a combination? We often recommend a dual option plan with both an inexpensive option and a rich benefit option.
- What is your current coverage, if any?
Note that we also work with dental, disability, life, pensions, and all other benefits plans. We also have a casualty division that handles all personal insurance and business insurance needs.
